Once there, click on ‘I’M A NEW CLIENT’ and you will get a link emailed to you so you can enter your personalized portal and request an appointment.
Staff will process your request and you will receive an email with a link to the forms and questionnaires.
Once they are completed, you will receive notification that your request was accepted.
Returning Patient Information Portal

Once there, click on ‘I’M AN EXISTING CLIENT’ and you will get a link emailed to you so you can enter your personalized portal and request an appointment.
You will receive an email once your request has been accepted or denied.
Alternately, you can download the app Client Portal to access your portal to request appointments, send messages, and pay invoices.
