New Patient Information Portal

  • The button below will take you to our patient care portal.

  • Once there, click on ‘I’M A NEW CLIENT’ and you will get a link emailed to you so you can enter your personalized portal and request an appointment.

  • Staff will process your request and you will receive an email with a link to the forms and questionnaires.

  • Once they are completed, you will receive notification that your request was accepted.

Returning Patient Information Portal

  • The button below will take you to our patient care portal.

  • Once there, click on ‘I’M AN EXISTING CLIENT’ and you will get a link emailed to you so you can enter your personalized portal and request an appointment.

  • You will receive an email once your request has been accepted or denied.

  • Alternately, you can download the app Client Portal to access your portal to request appointments, send messages, and pay invoices.
